ANSWER:

Fasting is first of all an attitude of sacrifice to create an expectation of God's grace and also so save money to offer as alms. For someone with a medical condition, fasting should be discussed with one's physician, and there might be other ways to eat what is less pleasant "bitter herbs!" and save money to offer as alms.
